MVP Vermont Smoking Cessation Program
MVP Keeping You Healthy - Smoking Cessation Benefit
Instructions on how to access the benefit:
- Congratulate yourself for the decision to quit smoking.
- For employer groups that have the MVP pharmacy benefits, MVP will cover generic Zyban® (bupropion) or Chantix* Smoking cessation medications have been proven to increase your chances for long-term quitting by forty to fifty percent.
- Call your primary care physician and ask to talk with him or her about quitting smoking. Let your physician know that you would like to take the generic form of Zyban® (bupropion) or Chantix.

- In addition to writing prescriptions, your doctor will need to fill out a pre-authorization form for this program. The preauthorization form gives approval for one month of the prescription. Before the end of the first month, you must call your physician's office to let him or her know that you are still motivated and that you have quit. Your doctor may then authorize a subsequent two-month approval of bupropion.
- Decide on a quit date to give up smoking, and give yourself plenty of time to obtain prescriptions before that date. Don't wait for the last minute. If your doctor wants to see you, you will need time to get an appointment. Also, the pre-authorization must come into MVP and be processed before you can pick up the prescription at the pharmacy. Your doctor and you will receive an authorization letter when the pre-authorization has been processed.
- When you receive the letter, you may take the prescription to your pharmacy. You are responsible for the copays for the generic version of bupropion.
- You can use bupropion and a nicotine replacement product at the same time.If you want to take a nicotine replacement product,(nicotine patches or gum), call the Vermont Quit Line (1-877 yes-quit or 1-877-937-7848). You will recieve a Quit Bucks voucher qualifying you for up to a ten week supply of the patches or gum. By presenting a Quick Buck at a participating pharmacy, you can purchase a two-week supply.
- The Vermont Quit Line provides helpful over-the-phone counseling. Every two weeks, if you continue to participate, you can get another Quit Buck voucher for another two weeks.
- Quitting smoking is one of the best things you will ever do for yourself.
*The maximum benefit is for two 3-month authorizations per calendar year. No more than 60 generic bupropion or Chantix tablets will be allowed per month. Your doctor can find the pre-authorization form on the Providers Home page of the MVP Web site.
