MVP Asthma Care Program

MVP can help you control your asthma through the Asthma Care Program.This program is designed to assist you or your child in managing asthma and to help prevent flare-ups from occurring. You will receive an informative newsletter twice a year regarding asthma, a free peak flow meter and educational mailings targeted to your specific needs, plus your child will receive a school asthma action plan.
If your asthma is not well controlled, a Registered Nurse is available as your personal Health Coach. Your Health Coach will assist you to make lifestyle changes that will help you to minimize triggers, maintain an appropriate medication routine and obtain control of your asthma.

Health coaching is an innovative solution to help you get on track and stay there. Health Coaches work with you and the rest of your medical team to provide support to help you improve your health and well-being.

Your Health Coach:

  • supports you
  • helps to motivate you
  • identifies your strengths and helps you build on them
  • works with you to discover the way to achieve your goals
  • helps you to see what may be getting in the way of your goals and how to overcome this.
Health Coaching will help you find many personal qualities that will assist you in making positive changes to manage your health. You will find these telephonic sessions to be exciting and rewarding, but the real accomplishments come from what you do between the coaching sessions.
A Health Coach is your own personal coach to help you reach your goals of good health. A personal trainer for your health!

What are the keys to making changes that will improve your asthma health?

  • Set realistic, measurable goals.
  • Determine how important those goals are to you.
  • What are the benefits of achieving these goals?
  • Determine your confidence level to achieve these goals.
  • What do you need to have in place to achieve your goals?
  • What information or education do you need?
  • Have a plan to deal with barriers as they arise.
