The Cardiac Care program is designed for members who are recovering from a recent heart attack, or other cardiac events such as a percutaneous coronary angioplasty or a coronary artery bypass graft (also called open-heart surgery). You will receive an informative newsletter regarding heart disease twice a year, reminders to obtain important blood tests and educational mailings targeted to your specific needs. You can also talk to a Health Coach to help with your Heart Healthy goals.

Health coaching is an innovative solution to help you get on track and stay there. Health Coaches work with you and the rest of your medical team to provide support to help you improve your health and well-being.

Your Health Coach:

• supports you
• helps to motivate you
• identifies your strengths and helps you build on them
• works with you to discover the way to achieve your goals
• helps you to see what may be getting in the way of your goals and how to overcome this.

A Health Coach is your own personal coach to help you reach your goals of good health. A personal trainer for your health!
