MVP Diabetes Care Program

MVP's Diabetes Care program was created to assist members in making healthy lifestyle changes and provide infromation and strategies to help better manage their diabetes. You will receive an informative newsletter regarding diabetes twice a year, a personal checklist of the recommended tests needed to help keep your diabetes in good contral, reminder cards for getting your eye exam, free glucometers and educationsl mailings targeted to your specific needs.
For members with an A1c level of 8% or greater, a Registered Nurse is avialable to help you make lifesyle changes that will help you to get your A1c level to the American Diabetes Association recommendation of less that 7%.
Health coaching is an innovative solution to help you get on track and stay there. Health Coaches work with you and the rest of your medical team to provide support to help you improve your health and well-being.

Your Health Coach:

  • supports you
  • helps to motivate you
  • identifies your strengths and helps you build on them
  • works with you to discover the way to achieve your goals
  • helps you to see what may be getting in the way of your goals and how to overcome this.
Health Coaching will help you find many personal qualities that will assist you in making positive changes to manage your health. You will find these telephonic sessions to be exciting and rewarding, but the real accomplishments come from what you do between the coaching sessions.

A Health Coach is your own personal coach to help you reach your goals of good health. A personal trainer for your health!

Taking care of yourself
When you have diabetes, there are things you can do to take good care of yourself.
Keep regular checkups with your doctor.

  • You should see your doctor at least four times per year.
  • Your blood pressure should be taken at every visit.
  • Your feet should be checked at every visit.
  • You should have a dilated eye exam once a year.
  • You should be monitored for kidney disease at least annually by having a microalbumin urine test.
  • You should have your LDL cholesterol monitored.
  • You should have a A1c done four times each year.
