As we begin April, we recognize Stress Awareness Month, an important time to help Members recognize and manage stress-related behavioral health concerns. Chronic stress can contribute to various behavioral health conditions, including anxiety, depression, and adjustment disorders, impacting overall well-being. According to the American Psychological Association, chronic stress has been linked to an increase in chronic illnesses, with 58% of adults aged 35 to 44 reporting such conditions in 2023, up from 48% in 2019. As health professionals, it is necessary to stay informed on best practices while ensuring accurate documentation to guarantee proper diagnosis and treatment planning for behavioral health care.

Key Documentation Tips

  • Be specific in diagnoses - Include details such as severity (mild, moderate, severe), episode (single or recurrent), and associated symptoms
  • Link symptoms to a diagnosis - Document how symptoms impact daily functioning
  • Include treatment plans - Outline interventions, medications, and follow-up plans to support medical necessity
  • Use standardized screening tools - Tools like PHQ-9 (depression) or GAD-7 (anxiety) strengthen documentation
  • Avoid using vague terms (e.g., using “depression” instead “major depressive disorder, moderate”)
